Duovir N Tablets are a combination of three drugs namely zidovudine, lamivudine and Nevirapine that are commonly used in the management of the Human Immunodeficiency Virus (HIV) infection. Zidovudine and lamivudine belong to the nucleoside analogue class of antiretroviral drugs which act by terminating the growth of the DNA chain and inhibiting the reverse transcriptase (RT) of HIV. Nevirapine is a non-nucleoside reverse transcriptase inhibitor (NNRTI). It acts by directly inhibiting Reverse Transcriptase.

Working of the drug:

Lamivudine and Zidovudine inhibits reverse transcriptase of HIV and prevents the proper transcription of the viral DNA. They both inhibit the HIV reverse transcriptase enzyme competitively and act as a chain terminator of DNA synthesis. The chain elongation process is inhibited and finally the viral DNA growth is inhibited. The NNRTIs or Nevirapine attach to and block the HIV enzyme called reverse transcriptase.

Dosage and Administration

Duovir N (Lamivudine/Zidovudine/Nevirapine) film coated tablets contain 150mg of lamivudine, 300mg of zidovudine and 200mg of nevirapine. Administration is by means of the oral route. However, due to the risk of serious and potentially fatal skin reactions, your doctor will recommend a 2 week period (called a lead in dose) in which the active ingredients are administered separately in accordance with the instructions of your doctor.

If your doctor is satisfied that nevirapine is well tolerated, you may then be told to take this medicine. Doctors generally tell patients to take one tablet twice per day (only after the lead in period). In all cases, you must do exactly as your doctor tells you, and never exceed the recommended dosage.

Before you take this drug:

• Do not take this drug if you:
• Are allergic to this drug or any of the other ingredients of this medicine

Duovir-N – Contraindications

Duovir N Tablets are contraindicated in patients with previously demonstrated clinically significant hypersensitivity (eg, anaphylaxis, Stevens-Johnson syndrome) to zidovudine, lamivudine and Nevirapine or any inactive components of the product. It is also contraindicated in patients with moderate or severe (Childs Pugh Class B or C, respectively) hepatic impairment.

How should Zidovudine be used:

It comes as a capsule and syrup to take by mouth, with or without food. Adult- PO- The recommended dose is 600mgday in divided doses.

Common side effects of Zidovudine :

Headache. Upset stomach or throwing up. Many small meals, good mouth care, sucking hard, sugar-free candy, or chewing sugar-free gum may help. Not hungry. Anemia and low white blood cell count.

What do I do if I miss a dose

Take a missed dose as soon as you think about it. If it is close to the time for your next dose, skip the missed dose and go back to your normal time. Do not take 2 doses at the same time or extra doses. Do not change the dose or stop this drug. Talk with the doctor.

What precautions should I take when taking Zidovudine :

If you have an allergy to zidovudine or any other part of this drug. Tell your doctor if you are allergic to any drugs. Make sure to tell about the allergy and what signs you had. This includes telling about rash; hives; itching; shortness of breath; wheezing; cough; swelling of face, lips, tongue, or throat; or any other signs.
